MPC FOCUS

The mutant prevention concentration (MPC) offers a novel approach to the treatment of infectious diseases. A therapeutic approach that takes into account the MPC of an antimicrobial can reduce the likelihood for selection of bacterial resistance as it favours a dose selection that not only aims at clinical cure but also at reducing selection of mutant bacterial strains1,2

WHAT IS THE MPC?

  • In large bacterial populations, spontaneous genetic mutations can occur; this phenomenon cannot be prevented
  • These mutations occur at a rate of around 1 in 10 million (107) bacteria3; hence in clinical infections where this size of infection is present first step mutants will naturally be present7
  • These mutant cells may be less susceptible to the antibiotic being used. The result can be that the resistant strain is selected for and will multiply1,2

Cmax: Peak level of antibiotic achieved in the serum of the animal with normal therapeutic dosing.

The bars represent the Mutant Prevention Concentration(MPC) value for each antibiotic. This is the amount of antibiotic needed to kill both susceptible bacteria and bacteria that have spontaneously mutated.

Where the bars turn dark purple beyond the Cmax, this indicates that the amount of antibiotic needed to reach MPC (and thus kill spontaneous mutants) is not achieved during normal dosing with the fluoroquinolone specified.

Compared to other veterinary fluoroquinolones, Veraflox® is the least likely to select for resistance14

  • By achieving drug concentrations that exceed the MPC, the potential for selection of resistant bacteria is reduced 6,7,8
  • With the lowest available MIC and MPC fluoroquinolone values overall, Veraflox® fights both susceptible and first-step resistant bacteria at therapeutic concentrations5
  • MIC (minimum inhibitory concentration) testing does not determine the amount of antimicrobial required to kill these mutant strains because it only looks at 100,000 or 105 bacterial colony forming units; so the population assessed isn’t large enough5
  • MPC testing determines the concentration of antimicrobial required to kill both susceptible bacteria and first-step resistant bacteria as it uses a larger inoculum of bacteria (109 CFU/ml); allowing an assessment of the capability to kill first step mutants when they will have appeared in the population9
  • Antimicrobial dosing based on MIC, and not MPC, could lead to under-dosing and the selection of resistant mutant strains1,2 By achieving drug concentrations that exceed the MPC, the potential for selection of resistant bacteria is reduced1,2,4

Veraflox® exceeds the MPC at the licensed therapeutic dose meaning it is less likely to select for resistant bacteria than other veterinary fluoroquinolones as it fights both susceptible and first step resistant bacteria
